Você tem mais de um problema por exemplo:

Indica que você tem entradas duplicadas na lista de sites proibidos, e o algoritmo do squid não permite isso, em versões mais antigas ele só te dá uma aviso, em versões mais modernas do squid ele pode gerar um erro fatal e não inicializar.
O seu segundo problema é que o seu diretorio de cache está cheio e não tem espaço pra gravar, o seu parametro de tamanho na linha "cache_dir" pode estar muito grande para o dispositivo você pode reduzir esse valor para dar alguma folga de espaço em disco, ele é expresso em Megabytes, segundo a documentação oficial.
Depois da alteração esvazie seu diretorio de spool e recrie o cache com o comando squid -Z.
